i'm looking to identify this mysterious fan, but need some help. Blades did not look like that originally, I litely polished them. Also has a worm gear on the back end, a shaft from the motor to the knuckle for oscillation, but some has bent a nail in it to stop it from free rotating, missing gear box?

I was able to find a picture of the badge, but the small text is impossible to discern. Any help would be appreciated
