I'm trying to find a manufacturer that makes a suitable replacement motor for this fan. Does anyone know where I can find the information needed to cross reference? I believe this model has the hex shaped cover. Thank you as always for trying to help all us trying to preserve these older fans.

I believe Grainger might have a suitable replacement motor in their Dayton line of electric motors. Just go to their webite and look through their electric motor section, and they should have a cross-reference for their motors to other manufacturers.
So what's wrong with the original motor that you can't use that anymore?
If the motor just isn't starting up or just humming when you turn it on, you more than likely have a bad motor run capacitor which the motor isn't gonna start or run without a working capacitor, so with that in mind your original motor could very well still be fine yet, but I'll need you to tell me what exactly the original motor is doing in order to know for sure if the original motor is indeed still salvageable or if you indeed really need a new motor.

Hope this helps.

Hex motor is not a capacitor motor.
